Thank you for expressing interest in seeing me as a new patient in my consultative practice. I schedule appointments Mondays and Wednesdays from 9:00 a.m. to 5:00 p.m. Please complete a copy of the Patient Agreement and mail it to me at:

Please keep a copy for your records. Appointments will be scheduled when your Patient Agreement is received. Call Betsy at 867-4114 to schedule an appointment. The length of the appointment is entirely at your discretion. I suggest one hour for your initial visit. Email correspondence will be an option only after you have seen me once in person.

The door to the office will be open if I am not in consultation with another patient. If the door is closed, please knock on the door and then announce your arrival by stating “your [scheduled appointment time] is here.” You may state your name if you wish. I will acknowledge your knock with a “thank you.” Upon arrival please complete the “Today’s Visit with …” sheet available on the clipboard in the wall pocket to the left of the entrance to the room. You may sit on the bench against the wall, or feel free to browse through the books and supplements on the shelves. I will call you into the consultation room myself, as soon as I am free. When you knock on the door to announce your arrival, I will let you know approximately how long a wait to expect.
