Thank you for expressing interest in seeing me as a new patient in my consultative practice. I schedule appointments on Mondays and on Wednesday afternoons. Please complete a copy of the Patient Agreement and mail it to me at:

Please keep a copy for your records. Call Betsy at 867-4114 to schedule an appointment. The length of the appointment is entirely at your discretion. I suggest one hour for your initial visit. Email correspondence will be an option only after you have seen me once in person.
In addition to this office website, I have an educational website, www.buffalo.edu/~shlevy/.
Please bring to your initial visit the actual bottles for each prescription medication and each supplement you are taking. This allows me to evaluate important information, such as dosage of each ingredient, as well as manufacturer.
The door to the office
will be open if I am not in consultation with another patient. If the door is
closed, please knock on the door and then announce your arrival, either by
stating your name, or stating your scheduled appointment time. I will acknowledge
your knock by letting you know the approximate wait time. Please complete the “Today’s
Visit with …” sheet available on the clipboard leaning against the corner
of the door. While waiting, you may sit on the bench against the wall, or feel
free to browse through the books and supplements on the shelves. I will call
you into the consultation room myself, as soon as I am free.
