Information For Prospective Patients

Thank you for expressing interest in scheduling as a new patient in my consultative practice. I see patients in the office on Mondays and Wednesdays (following the initial in-person visit, I offer phone and email follow up 5 days per week).

A Patient Agreement must be submitted in advance of the initial visit. Please click here to complete the Patient Agreement.

Please call Naurene at (716) 867-4881 to schedule an appointment. The office phone is monitored/voice messages are returned from 8:00 am - 5:00 pm Monday-Friday and from 9:00 am - noon on Saturday. I suggest scheduling for one hour for your initial visit.

Bring to your initial visit the bottles/containers for each prescription medication and supplement you are taking. This allows me to determine the dosage of each ingredient, as well as manufacturer.

Prior to your visit, think about your reasons for seeing me, your goals for the visit, and any questions you might have, and write these down.

Please arrive 5 minutes in advance of your scheduled visit and complete the intake form (with a title of “Intake Form for initial visit with Sanford Levy”) on the clipboard on the first counter as you enter the office. Alternatively, you can print and complete this one-page intake form in advance of your arrival by clicking here and scroll to the bottom of the page and click on “PRINTABLE INTAKE FORM.”

I will personally meet you in the waiting room to take you into the consultation room. You are welcome to bring a family member or a friend.

Payment is required in full at the end of each office visit - the charge is based on the amount of time used. Payment by check or cash is requested; I do accept MasterCard, Visa, American Express, and Discover credit cards, flexible spending account and HSA debit cards. I do not accept insurance-company issued wellness cards.
